Lieutenant-General Mthombeni, the Provincial Commissioner of Gauteng, appears in the commission’s record both as a witness and as a figure whose actions drew scrutiny. He also faced questions about contact numbers he provided, including one for Lieutenant Mawela, though he could not recall whether the official, referred to as Mashaba, actually phoned that number.
