Colonel Phillips

Colonel Phillips served in the Inspectorate, the internal oversight arm of the police, and was tasked with reviewing complaint dockets and summarising his findings in written reports. During the commission’s proceedings, his handling of a specific docket—referred to as CAS 38—came under scrutiny. Testimony described how Phillips would examine a docket, note any problems he found, and then stop his inquiry if he could not identify a link to the alleged cover-up involving General Khumalo. He faced questions about whether a deeper investigation of that docket would have uncovered evidence of such a cover-up, and about other investigative leads he was expected to pursue but did not.
